Traveler's Checklist
Before Traveling
□ Get authorization for travel expenses from the Legatum Center. Note: all grant recipients are already authorized to purchase the items on their proposal. Any significant changes to the proposal require authorization.
□ Please fully explore the SOS website BEFORE traveling to understand the policies of the International SOS medical and security evacuation policies and your qualifications for this program.
□ Determine if you are traveling to a moderate risk, high risk or highest risk country. If so, please read the information below.
□ If you are traveling to a country where vendors are not accustomed to giving receipts, we encourage you to stop by the Legatum Center offices and pick up a receipt book to bring with you.
Keeping track of expense receipts
□ Keep all original receipts (if you purchase through any of the three approved MIT travel agencies, they will bill our account and send us the necessary paperwork).
□ Keep all boarding passes and submit them to the Legatum Center.
□ For taxi receipts, please fill in the “To”, “From”, and "Dates" fields and include the tip as part of the amount on the receipt.
After completing your travels
□ Go to http://vpf.mit.edu/site/travel/forms and fill out the travel expense voucher. Please call 617-324-1875 if you have any questions on how to fill it out. When you have completed the form, please print and submit it (along with your boarding passes) to the Legatum Center. To see a sample form with the required fields highlighted, click here. Do not fill in your social security number. Only fill in the highlighted sections that apply to your trip.
□ Send in your report, photos and video.

Travel Risk Levels
If you are determined to be traveling to a country with a "moderate", "high", or "highest" travel warning, please be sure to adhere to the following requirements. For more information, please consult the MIT Travel Risk Policy.
- If student is over 18: Sign the travel risk form
- If student is under 18: Sign the travel risk form and get parental permission
- If the student is from the country he/she is traveling to: Sign the travel risk form
- If student is from the country he/she is traveling to and under 18: Sign the travel risk form and get parental permission
- If the student is not from the country he/she is traveling to: Sign the travel risk form and ask the Chancellor for a travel waiver
- If student is not from the country he/she is traveling to and under 18: Sign the travel risk form and get parental permission
- If the student is from the country he/she is traveling to: Sign the travel risk form
- If student is from the country he/she is traveling to and under 18: Sign the travel risk form and get parental permission
- If the student is not from the country he/she is traveling to: Sign the travel risk form and ask the Chancellor for a travel waiver
- If student is not from the country he/she is traveling to and under 18: Sign the travel risk form and get parental permission
